The objectives of Sudbury’s Centre for Transitional Care (SCTC) are to build capacity within the homeless serving sector.We use service integration methods to make resources provided by independent and government-run organizations readily accessible. The goal of integrating these services is to meet each individual where they are at, complete a risk assessment, and develop a personalized plan to support individuals in supporting themselves and dependents.
